In short
Hospital discharge support helps NDIS participants move safely from hospital to home or supported accommodation. We work with you, your family and the hospital team to plan the move, so care, equipment and services are in place when you leave.
Who it’s for

- Families who want help planning a safe return home
- Hospital discharge planners, social workers and support coordinators
- People whose support needs have changed after a hospital stay

01
How the NDIS helps when you’re in hospital.
Let the NDIS know when you go into hospital. With your consent, hospital staff can also tell the NDIA. The NDIA has staff who work with hospitals to share information and help plan your discharge.
If your support needs have changed, your plan may need to change too. You can usually keep your support coordinator or recovery coach while you’re in hospital, so they can help get things ready for when you leave.
02
A discharge checklist for families.
Before the discharge date, it helps to check:
- Is the discharge date confirmed, and who is picking you up?
- Are medications, scripts and any changes explained?
- Is equipment – like a shower chair, hospital bed or hoist – ordered and delivered?
- Is the home safe and easy to move around?
- Are follow-up appointments booked?
- Who is supporting you on the first night and in the first week?
- Do the NDIA and your support coordinator know about the discharge?
We’re happy to work through this list with you and the hospital team.
Not sure where to start?
Tell us what you need. We’ll explain your options for hospital discharge and check what your NDIS plan can cover.
03
When you can’t go straight home.
Sometimes home isn’t ready – maybe modifications are still being done, or you’re waiting for a new long-term home. Short-term or medium-term accommodation can bridge the gap, with support included. If you need a home built for very high support needs, read about Specialist Disability Accommodation (SDA).
04
The first weeks at home.
Many people need more support in the first days and weeks after hospital, then less as they get stronger. We review your support as you recover and adjust it with you. If you still need wound care or help with medications, our registered nurses can visit at home.
NDIS funding
How it’s funded.
If you’re an NDIS participant, support after leaving hospital may be funded through your plan when it relates to your disability. If your needs have changed a lot, you may need a plan variation or a plan reassessment – we can explain the options and help you get started.

FAQ
Hospital Discharge: your questions.
Still not sure? Call us or send a message and we’ll help you work it out.
When should we contact you?
As early as possible – ideally as soon as a discharge date is being discussed. That gives us time to plan the right support.
Can hospital staff refer someone to you?
Yes. Discharge planners, social workers and support coordinators can call or email us directly.
Can I go to supported accommodation after hospital instead of home?
Sometimes that’s the safest option. We offer short-term and medium-term accommodation, and we can talk through what suits your situation and your plan.
What if my needs have changed since my plan was made?
Your plan may need to change. We can explain the difference between a plan variation and a plan reassessment and help you take the next step.
Does the NDIS fund support while I’m still in hospital?
Hospital staff generally provide your care while you’re in hospital. In some situations, such as complex communication needs or behaviours of concern, the NDIS may fund disability supports during a stay. Your support coordinator or recovery coach can usually keep working with you.
What if my equipment or home modifications aren’t ready?
Short-term or medium-term accommodation may help while you wait. We can talk through the options with you and your support coordinator.
Who should tell the NDIA I’m in hospital?
You, your family or your support coordinator can tell the NDIA. With your consent, hospital staff can tell them too.
